Data control
For the purposes of the Data Protection Legislation (General Data Protection Regulation as well as any other laws, regulations, standards and codes relating to data protection), Electrify Marketing & Communications (Electrify) is the data controller (Registered address: 1C Alba Pavilions, Alba Business Park, Livingston, EH54 7HG).
This consent form is created with Jotform Inc. (111 Pine St. Suite 1815 San Francisco, CA 94111, USA). Electrify and Jotform have engaged in a signed data processing agreement (DPA).
Project details
For this project, Electrify is working on behalf of The Scottish Government to deliver ongoing creative content for Suicide Prevention Scotland and key partners including Samaritans Scotland, Scottish Action for Mental Health (SAMH), Change Mental Health, Penumbra Mental Health, Public Health Scotlandand other delivery partners as required from time to time.
Purpose of consent
The film, audio and images from this project will be used for any of the above partners on their:
- Websites
- Social media channels - including, but not limited to Twitter/X, Facebook, Instagram, Linkedin, YouTube
- Documents - such as information leaflets, reports, brochures, flyers, both digitally and in print
- Archive - images or recordings may be kept permanently once they are published as an archive of this project.
You should note the following information:
- Images and films on websites can be viewed throughout the world and not just in the United Kingdom
- There some overseas countries may not provide the same level of protection to the rights of individuals as EU/UK legislation provides
Managing your data
The personal data collected on this form will be held securely and will only be used for administrative purposes.
You have the right to request to see a copy of the information we hold about you and to request corrections or deletions of the information that is no longer required.
You can ask Electrify to stop using your images at any time, in which case it will not be used in future publications but may continue to appear in publications already in circulation.
If you have concerns about the use of your image this should be raised in writing via data@electrify.me.uk.
You have the right to lodge a complaint against Electrify Marketing & Communications regarding data protection issues with the Information Commissioner’s Office via ico.org.uk/concerns.
